TMBS CONSENT & COMPLIANCE ADDENDUM

This addendum outlines important consent, acknowledgment, and authorization requirements for all new patients receiving telehealth-based peptide, GLP-1, or weight management care through Trilogy Mind Body Soul, LLC. Please review each section carefully and sign at the end to confirm understanding and consent to treatment.

Section A: Telehealth Treatment Consent

I voluntarily consent to receive medical care, diagnosis, and treatment through telehealth technologies. I understand that telehealth involves electronic communication and may limit physical examination. I understand I may withdraw consent at any time, and that emergency services must be contacted by dialing 911 if urgent conditions occur. I understand that my provider will maintain confidentiality consistent with HIPAA and state law.

Section B: HIPAA Notice & Privacy Practices Acknowledgment

I acknowledge that I have received and reviewed the Trilogy Mind Body Soul HIPAA Notice of Privacy Practices and Privacy Policy. I understand my rights regarding access, correction, and confidentiality of my protected health information (PHI). I authorize TMBS to use encrypted communication systems for telehealth visits and consent to electronic exchange of medical information under HIPAA compliance.

Section C: GLP-1 & Peptide Medication Consent

I understand that GLP-1 medications (e.g., Semaglutide, Tirzepatide) and peptides may be prescribed based on medical evaluation and lab results. I understand that some therapies may involve off-label use or compounded formulations prepared by licensed 503A pharmacies. I agree to follow all provider instructions, complete required lab work every four months, and immediately report any adverse effects such as nausea, vomiting, severe abdominal pain, or other concerning symptoms.

Section D: Refund & Returns Policy Acknowledgment

I understand that once medication is prescribed, dispensed, or shipped, it cannot be returned or refunded in accordance with FDA and pharmacy regulations. Consultation and telehealth visit fees are non-refundable after services are rendered. Any disputes or errors will be promptly reviewed by TMBS support staff.

Section E: Payment & Communication Authorization

I authorize Trilogy Mind Body Soul, LLC to charge the payment method provided for telehealth services, consultations, and medication processing fees. I acknowledge that all services are self-pay and not eligible for insurance reimbursement. I consent to receive electronic communications (email, SMS) regarding scheduling, prescriptions, and follow-up reminders.

Section F: Acknowledgment & Signatures

By signing below, I certify that I have reviewed, understood, and consented to all sections of this Intake Packet including telehealth consent, HIPAA acknowledgment, treatment consent, refund policy, and payment authorization.
